A summary of the health, welfare, and financial savings programs available to newly hired employees is shown below. The specific benefits an employee is eligible for varies by location and employee group. Additional information about these programs, an employee’s share of the cost, and applicable benefit waiting periods, are provided once an applicant is hired.*
Medical coverage
SuperMedia’s medical plan options provide comprehensive coverage for preventive care, doctor visits, hospital care, and prescription drugs. Employees and SuperMedia share the cost of medical coverage.
Dental coverage
SuperMedia’s dental plan options cover preventive, basic and major dental procedures. Some dental plan options also cover orthodontic services. Employees and SuperMedia share the cost of this coverage.
Vision benefits
Vision benefits are included when an employee is enrolled in a medical plan option. Eye exams, glasses and contacts, and discounts on other vision related services such as LASIK are included in the vision program.
Flexible spending accounts
SuperMedia makes available Health Care and Dependent Day Care flexible spending accounts that help employees save tax dollars on eligible expenses.
401(k) savings plan
SuperMedia offers a 401(k) savings plan with a wide range of investment options. The company matches a portion of an employee’s contributions to this plan. Employee contributions may be made on a pre-tax, after-tax and/or Roth 401(k) basis.
Short term disability
SuperMedia’s short term disability program provides temporary, partial income replacement when a qualifying illness or injury occurs. Employees are automatically enrolled in this program when eligible. SuperMedia pays the full cost of this coverage.
Long term disability (LTD)
Long term disability coverage is also available. The LTD program provides additional income protection when an employee is unable to work for an extended length of time due to a medically-approved disabling condition. Employees pay the full cost of this coverage.
Life insurance
SuperMedia provides life insurance coverage equal to one times pay. Employees can also purchase supplemental life coverage on themselves and/or their eligible dependents. Some supplemental coverage requires evidence of insurability.
Accidental death & dismemberment (AD&D) insurance
SuperMedia provides AD&D insurance coverage equal to one time pay. Employees can also buy supplemental AD&D coverage on themselves and their eligible dependents.
Business-travel accident insurance
Employees are automatically provided business-travel accident coverage. This coverage provides additional protection when travelling on SuperMedia business.
Employee assistance program (EAP)
Employees may receive confidential and personal counseling assistance to help deal with challenges that can affect health, relationships or job effectiveness through SuperMedia’s EAP. The EAP is also there to assist employees with legal and financial challenges, work/life services, adoption assistance, and tobacco cessation. SuperMedia pays the cost of EAP coverage.
Time off
SuperMedia’s time off programs include holidays, vacation, and personal days. In addition, SuperMedia is closed between Christmas and New Year’s Day.
Alternative work arrangements
SuperMedia makes available alternative work arrangements, such as telecommuting and compressed work weeks, to help employees balance work with other activities, personal needs, family, and interests outside of work.
